How our Behaviour Support Practitioner model works
The process is collaborative and structured, beginning with understanding the person and progressing to practical support planning and implementation.
Initial Assessment
A practitioner meets with the participant and gathers information from the people who know them well. Assessment considers behaviours of concern, routines, environments, communication, strengths and support needs so the team can develop a meaningful understanding of the person.
Positive Behaviour Support Plan
Following assessment, the practitioner develops a Positive Behaviour Support Plan with practical guidance for participants, families and support teams. Strategies focus on meeting needs, building skills, supporting quality of life and reducing reliance on behaviours of concern.
Transforming lives through Positive Behaviour Support
Behaviour support is not simply about reducing a behaviour. It is about understanding what matters to the person and creating practical supports that help them participate, communicate and achieve meaningful goals.
Understanding existing routines, values, relationships, preferences and individual strengths.
Understanding behaviours of concern and the circumstances in which they occur.
Identifying needs and developing new ways for those needs to be communicated and met.
Building positive behaviours and practical skills that support the person's own meaningful goals.
